The St.Thomas Optimist Softball Association will maintain the above information for a period of three years. This information will not be released to any persons, companies or organizations not directly affiliated with the St. Thomas Optimist Softball Association. The maintenance of the above information may remain in its hard copy (paper) form or may be electronically stored for future reference, but such storage will not exceed a three-year period. At the conclusion of three years, said documents (hard copy or electronic) will be appropriately destroyed. I understand that there is a risk in the sport of softball and give permission for my child to participate in the activity. I will in no way hold any member of the St. Thomas Optimist Softball Association or executive responsible for loss or injury occurring during the participation of the sport.

CHEQUES MADE PAYABLE TO OPTIMIST SOFTBALL

THERE IS A $20 FUNDRAISING FEE, MAX $40 PER FAMILY, PAYABLE WITH REGISTRATION, TICKETS WILL BE AVAILABLE AT REGISTRATION

Players are encouraged to play in their own age division, a player may move up but not down in division

Fees apply to players who are of the SAME IMMEDIATE FAMILY.
